Tax filing can feel daunting and stressful whether it’s annual or quarterly. We’d like to think it doesn’t have to be.

Help us help you by providing these requirements.

NEW ACCOUNT DETAILS

1. FULL LEGAL NAME  2. SOCIAL SECURITY NUMBER 3. DATE OF BIRTH for each person that appears on the tax return. This includes: (taxpayer, spouse, and dependents). (Please note that W2s and 1099s tend to obscure this information on the forms to help prevent identity theft.) 4. MAILING ADDRESS for the tax form and where to send completed tax for review and signatures. 5. PHONE / EMAIL where you can be reached. A message can be left if you are not available. 6. COPY OF PREVIOUS YEARS TAX. This helps expedites your filing as it’s often needed as a reference. 7. Upon completion, your filing will be mailed for review. Please return the signature pages with payment, keep the remaining pages for your records. Additional copies available upon request [$25 fee]

CHECKLIST FOR TAX FILING 

Below is a partial list of items you may have

INCOME

  1. W2 from employer or W2-G for gambling winnings.
  2. 1099s from IRAs or pensions (1099-R), investment income such as interest (1099-INT), dividends (1099-DIV or stock sales (1099-B or 1099-Composite)
  3. Social Security (1099-SSA)
  4. Unemployment and Paid Family Medical Leave (1099-G)
  5. For self-employed individuals (1099-NEC and 1099-MISC) along with a compilation of any income not included on the 1099 forms
  6. Rental Income (1099-MISC) and compilation of income not reported on a form.
  7. Payments from payment cards, apps or online marketplaces such as EBay or PayPal (1099-K)
  8. Property Sale (1099-S)
  9. Alimony
  10. K-1 forms from partnerships, S-Corps, estates, and trusts.

EXPENSES AND DEDUCTIONS

  1. Summary breakdown of job and overhead expenses associated with income from self-employment and rental properties
  2. Daycare expenses, provider information (Name, address, tax identification number, and total expenditure).
  3. College tuition expenses (1098-T)
  4. Student loan interest (1098-E)
  5. IRA contributions
  6. Medical and health care expenses
  7. Real estate, excise and school taxes
  8. Mortgage interest, points and mortgage insurance (1098)
  9. Donations to charities
  10. Gambling losses

ADDITIONAL ITEMS

  1. Massachusetts Residents – Proof of health insurance coverage (1099-HC or 1099-SSA for Medicare)
  2. Form 1095-A for people purchasing health insurance through the Health Connector or similar government programs.
  3. New York Residents – Copy of front and back of driver’s license or state identification card.
